Patient navigation is a process by which an individual—who knows about the health care system and available resources—guides patients through complex medical systems helping them negotiate and overcome barriers to care.
Many low-income, uninsured patients face not only financial barriers, but also cultural and linguistic barriers to accessing care. The Primary Care Coalition works with clinics participating in Montgomery Cares, as well as other safety-net clinics in neighboring jurisdictions, hospitals, and community health workers, to establish patient navigation for our most vulnerable neighbors.
Patient navigation is fundamental to improving access to high-quality health care and is incorporated into all PCC projects and PCC administered programs.
